Foundation-Supported Hospitals and Centres

We are proud of our widespread presence in the community, which covers 2 hospitals, 8 local community services centres (CLSCs), and 14 residential and long-term care centres (CHSLDs).

This variety and reach is what sets us apart and allows us to have a substantial impact on the health and well-being of the population throughout every stage of a person’s life.

The Verdun Hospital

Opened in 1932, the Verdun Hospital serves the Côte-Saint-Paul, Nuns’ island (Île-des-Sœurs), Little Burgundy (Petite-Bourgogne), Pointe Saint-Charles, Saint-Henri, Verdun, and Ville-Émard neighbourhoods.

Affiliated with the University of Montréal’s Family Medicine Department, the hospital is home to the third-largest family medicine centre on the Island of Montréal and provides outpatient services for day surgeries and day medicine.

The Verdun Hospital is currently undergoing expansion and modernization work to better meet the needs of the community.

Notre-Dame Hospital

The Notre-Dame Hospital has been in its current location, on Sherbrooke Street across from Lafontaine Park, since 1924.

In November 2017, it became a community hospital serving the boroughs of Ville-Marie and Le Plateau-Mont-Royal, which represent 140,000 residents and over 600,000 workers, students, and tourists.

It offers care for every stage of life—from birth to end-of-life—and treats general illnesses and medical conditions. Adopting an inclusive and welcoming approach, the hospital is very active in promoting urban health.

The Notre-Dame Hospital Pediatric Clinic is currently being renovated to better suit the needs of its young patients and their loved ones.

Residential Centres

CHSLDs are residential and long-term care centres for individuals who need constant surveillance and specialized care and who can no longer live at home in a safe manner.

The foundation supports projects in the 14 CHSLDs located on the CIUSSS du Centre-Sud-de-l’Île-de-Montréal territory. These projects focus on making CHSLDs pleasant and comfortable living environments for the people who reside there.

Community Service Centres

CLSCs are local community service centres that offer health and social services (e.g., prevention programs in schools, sexual health support for vulnerable individuals, at-home care for older adults, mental health social services, etc.)  in the centres themselves, but also in schools, workplaces, and in private homes.

Fondation Santé Urbaine supports the 8 CLSCs located on the CIUSSS du Centre-Sud-de-l’Île-de-Montréal territory:

  • CLSC Ste-Catherine (Faubourgs)
  • CLSC de la Visitation
  • CLSC Parthenais
  • CLSC Plateau-Mont-Royal
  • CLSC Saint-Louis-du-Parc
  • CLSC de Saint-Henri
  • CLSC de Verdun (as well as the point of service on Nuns’ Island (Île-des-Sœurs))
  • CLSC de Ville-Émard

The CIUSSS du Centre-Sud-de-l’Île-de-Montréal

The CIUSSS du Centre-Sud-de-l’Île-de-Montréal is a government organization that provides health and social services to the Centre-Sud-de-l’Île-de-Montréal neighbourhood, from birth to end-of-life.

It offers a wide range of services to a population of over 422,000 people and employs over 23,000 health-care professionals and social workers. Its goal is to provide high-quality patient- and family-centred services in partnership with local communities.

The CIUSSS du Centre-Sud-de-l’Île-de-Montréal sets itself apart through its innovative and effective health-care and social service initiatives and its leadership in social innovation.
